Brown Consultants (BC) began from a vision of one dedicated woman who believed that she could make a difference in the lives of individuals with developmental disabilities. Her dream was to create a program where individuals and their families could be heard. BCA continuouslly searches for ways to enhance the quality of life for its consumers, while assuring that all state regulations are in compliance. Our mission was developed with the core belief that all individuals have the right to grow and develop to their fullest potential. We believe in person centered planning and that the consumer and their familly or guardian should be at the center of all planning and implementation. BCA has stafff that are committed to our mission. Our staff recieves on-going training for program implementation and health and safety issues that will aid in the consumers personal growth and development.

| How to transfer from one program to another |
You will be notified by the Mental Retardation Authority (MRA) that you or your family member's name has come up on the HCS list and that you or your family member has a HCS slot. The MRA Service Cordinator will give you a list of providers that you may choose from to provide the services. If you choose us the following information will be requested from the service cordinator: - Medication history - Immunization record - Medical Historoy - Copy of social security card - Copy of birth certificate - Medicaid card - Copy of MRRC - Copy of ICAP - Copy of guardianship papers (if applicable) - Copy of current planning conferences - Copy of applicable assessments, i.e. psychological, dietary,etc. The Service Cordinator will cordinate with you and BCA a convenient pre-enrollment meeting time and place to further discuss the appropiate services for you or your family member. During this meeting, the above items will be reviewed by the Interdisciplinary team (IDT). An interdisciplinary team consists of a consumer, their legal guardian or family member, a case manager, a nurse and any other persons deemed neccessary. You may transfer HCS providers at any time. Your case manager will assist you in this process by providing you with a provider list. Once you have selected the provider of your choice, you will need to sign a tansfer request for the provider to obtain the neccessary paperwork regarding your care. Your current provider and the future provider will cordinate a meeting with you and your family members to finalize the transfer. You may be discharged from the program for the following reasons: - you become ineligible for services by the Medicaid program. - you or your legal guardian request discharge. - we can not assure your general health, safety and welfare. All permanent discharges must be authorized by Austin. Temporary discharges will occur for the following reasons: - Hspitializations (medical or psychiatric) - Furlough / vacations - Elopements (runaways) Temporary discharges do not result in a loss of placement.

| Adaptive Aids | | Counselling and therapy | | Day Habilitation | | Dental Services | | Minor home modifications | | Nursing | | Residential Assistance | | Respite | | Supportive Employment | Adaptive aids consists of the following services: repair and maintenance not covered by the warranty for some lifts, mobility aids, control switches, mecially neccassary supplies, positioning devices, enviromental control units, communicaition aids (including batteries), adapted equipment for activites of daily livin and safety devices. Approval for some adaptive aids requires a physician's order and / or justifcation from a certified therapist. some adaptive aids also require a prior authorization from austin prior to purchase. Components include:
- Audiology - dietary - Occfupational Therapy - Physical Therapy - Psyschology - Social Work - Speech / Language Daily habilitation includes: - Assiting individuals in acquiring, retaining and improving self-help skills, socialization abilities and adaptive skills neccessary to reside successfully in the community. - Reinforcing any skills or lessons taugh in school, therapy or other settings. - Providing training and support activities which promote the consumer's intergration and participation in the community. - Providing assistance for consumers who can't manage their personal care needs during day habilitation activities. - Providing transportation necessary for the individual's day habilitation activities. For individuals that are no longer Medicaid eligible for dental services , HCS pays up to $1000.00 per IPS year for dental care. This can cover preventive, emergency and thereutic treatments. There is a lifetime available to the consumer. The modifications must be required to enhance the consumer's quality of life. The modifications require approval from a certified therapist and prior approval from Austin. This can include:
-Purchase and repair of wheelchair ramps
-Modifications to bathroom -Modifications to kitchen -Specialized accessiblity and safety adaptions, including repair and maintenance. Nursing can include: - Administerting medication - Monitoring the consumer's use of medication - Monitory health data and information - Assisting in securing emergency medical services for the consumer - Making referals for appropriate medical services - Performing healthcare procedures ordered or prescribed by a physician and required by standards of professional practice or law to be performed by licensed nursing personel. - Delegating and monitoring tasks assigned to toher service providers by a registered nurse (RN) in accordance with state law. Supportive home living
Provied to consumers in their own residence or the residence of their family. Staff will come to the residence and provide hands on training based off recomendsations of the consumers interdisciplinary team. Foster / Companion Care Provided in a private residence by a fost or companion care provider who lives in the residence. A parent or other family member may provide this if the consumer is 18 years of age or older. The parent or family member must complete all required training and sumbit the required paperwork first. Residential Support Daily shif staff or individual staff provide support. This is available for consumers that must have 24 hour awake staff. Residential support includes the following: - Providing direct personal assistance with actices of daily living (grooming, eating, bathing , dressing, personal hygiene). - Assisting with meal planning and preparation. - Securing and / or providing transportation. - Assisting with housekeeping. - Assisting with mobility. - Reinforcing therapy services. - Assisting with medications and the performance of tasks as delgated by the RN - Supervising the consumer's safety. - Facilitating inclusion in the community activities, use of natural supports, social interaction and participation in leisure activities. - Provining habilitation, exclusive of Day habilitation. Supervised living Provides the same services as residential support with the exception of awake staff. Staff remains available, but may sleep. This service is provided to consumers residing in the residence of their natural or adoptive parents who are receiving supportive home livng (HCS) or community support (GR). Respite may be provied in or out of the home. Respite services are provided in hourly or daily increments. When hourly respite is provided in an increment of 10 hours or more a day, the service is converted to daily respite. HCS provides a maximum of 300 hours or 30 days per IPC year. Hourly and day services may be combined but must not exceed the maximum allowed. Respite services for General Revenu vary. these include :
- Assistance with training in self-help and independent living skills. - Provision of room and board when provided overnight in a setting other than the consumer's normal residence. - If respite is provided in a setting other than the consumer's normal residence , the family is responsible for providing personal items as such as diapers, any adaptive equipment needed; medications, etc. Note: A physician's order is needed to disperse medication. This Service is available to consumers that are being compensated by an employer in accordance with the Fair Labor Standards Act. - Provided as ongoing support services need to sustain paid work by teh consumer, including supervision and training. - Supported employment services is provided when these services are not available or funded through the state education agency or state rehabilitation agency.
